Your information will be automatically encrypted before being sent.  Of course, you may also fill out the intake forms at your first visit.  Please plan to arrive at least 20 minutes early, if you choose this option.

Our office is located next to Summit Hospital, between 29th and 30th streets, and between Broadway and Telegraph Avenue. Parking options include metered parking (quarters) on most streets, a multilevel pay parking garage (enter from 29th or 30th street), and limited free street parking on 29th and 30th streets, across Telegraph Avenue.
